What is Rhinoplasty?

Rhinoplasty is a surgical procedure to reshape the nose. It can address concerns such as a prominent hump, a droopy or bulbous tip, a crooked bridge or overall size that feels out of keeping with other facial features. In some cases, functional issues such as nasal obstruction can also be improved alongside cosmetic changes.

The operation is usually carried out under general anaesthetic. Depending on the technique, incisions may be hidden inside the nostrils (closed rhinoplasty) or combined with a small incision across the columella, the thin strip of skin between the nostrils (open rhinoplasty). Bone and cartilage are then carefully reshaped to refine the profile and tip, while maintaining structural support and nasal function. A small splint is often applied afterwards to support the nose as it heals.

What to Expect from Rhinoplasty Surgery

Consultation & Preparation

  • Assessment & Planning:
    At your consultation, Mr Andrew Pieri will assess your nasal shape, skin thickness and facial proportions. Clinical photographs from multiple angles are taken to aid planning and discussion. Together, you will explore which aspects of your nose you would like to change and what is realistically achievable, with the aim of preserving or improving function as well as appearance.

  • Pre-operative Guidelines:
    You will be advised to avoid medications that increase bleeding risk and to stop smoking well before surgery, as it can impair healing. Because rhinoplasty is typically performed under general anaesthetic, fasting from midnight is required, and you will be given clear instructions regarding your regular medications.

  • Practical Arrangements:
    You should arrange for someone to accompany you home and stay with you for at least the first night. It is sensible to have soft foods available, and to prepare a comfortable resting area with your head elevated, which helps minimise swelling after the operation.

Surgery & Immediate Aftercare

  • Anaesthesia & Procedure:
    Rhinoplasty usually takes 1.5–3 hours. Under general anaesthetic, incisions are made either within the nostrils (closed) or combined with a small incision across the columella (open). Bone and cartilage are reshaped to refine the bridge and tip, straighten the nose where possible and improve symmetry. The incisions are then closed with fine sutures. A small external splint is applied to support the nose, and soft internal dressings may be placed in the nostrils.

  • Dressings & Comfort:
    You can expect some nasal congestion, mild bleeding and pressure around the nose and eyes. Bruising around the eyes is common and usually fades over 1–2 weeks. Pain is generally mild to moderate and managed with pain relief. Keeping your head elevated and using cold compresses as advised can help reduce swelling.

Recovery, Results & Follow‑Up

  • Early Recovery (Weeks 1–2):
    You will be advised not to blow your nose and to avoid bending down or heavy lifting in the first week. The external splint is usually removed at around 7–10 days. By this time, most bruising has started to resolve, and many patients feel able to return to desk-based work.

  • Resuming Activities:
    Strenuous exercise, contact sports and activities that risk impact to the nose should be avoided for at least 4–6 weeks. Glasses should be used with care and may need to be supported differently initially to avoid pressure on the healing nasal bones.

  • Healing & Outcome:
    Swelling continues to improve over several months. Although the nose will look noticeably different once the splint is removed, refinement of the tip and profile carries on for up to a year. Scars, where present, are usually small and well concealed. The aim is for a nose that looks natural and harmonious with the rest of your face.

  • Follow-Up Appointments:
    You will be reviewed at around 1 week for splint and, if applicable, suture removal, with further reviews at 1–3 months and 6–12 months to monitor healing and fine-tune aftercare advice.
